Insurance Information

We take most insurances except for the following: Humana, Optimum, Freedom, GHI (CBP), United Healthcare SNP, and United Healthcare Community Program.

The following insurances require a referral from your Primary Care Provider or Referring Doctor prior to making an appointment: Blue Select HMO, Staywell, Wellcare, Medicaid, Aetna HMO, Tricare Prime (from VA, not PCP), and Ultimate. When we receive the referral we will call you to make an appointment.
